Missouri Primary Care Association Health Equity & Social Determinants of Health Coordinator Location: Main office Jefferson City, remote options within MO will be considered The Health Equity and Social Determinants of Health Coordinator is an individual who works with outpatient primary care practices on standardized screening for social and structural determinants of health (SSDH), referrals to community social service partners, team-based care, practice transformation to ensure meeting National Standards for Culturally and Linguistically Appropriate Services (CLAS) in Health and Health Care with goal to reduce and eliminate health and racial disparities for Missouri's most vulnerable citizens. • Designs and delivers individualized and group coaching/facilitation to Federally Qualified Health Centers for workflow implementation and optimization to increase standardized screening of patient social and structural determinants of health (SSDH) needs. • Designs, facilitates, and provides technical assistance and sharing of best practices with health centers to develop and enhances robust local/regional community social service referral sources and closing referral feedback loop to assist patients in meeting identified SSDH needs. • Designs, facilitates, and provides training, technical assistance, and practice transformation for member health centers and MPCA departments/programs to ensure meeting and complying with the latest requirements and guidance outlined by the National Standards for Culturally and Linguistically Appropriate Services (CLAS) in Health and Health Care. • Engages and facilitates information gathering from health centers, local/regional/state stakeholders, and MPCA departments/programs to identify challenges and opportunities to improve health and racial equity to inform MPCA leadership and assists with evaluating identified programming, policy, and funding needs.
